ABSTRACT

Objective:To investigate the effect and mechanism of short-chain fatty acids (SCFAs) on the side-effect of tacrolimus on blood glucose.Methods:The C57BL/6 mice were treated with tacrolimus orally (10 mg/kg, tacrolimus group), tacrolimus plus 150 mmol/L sodium butyrate and isovalerate mixed solution (SCFAs group), broad-spectrum antibiotics (antibiotic group), and tacrolimus plus broad-spectrum antibiotics (tac&abx group). After 8 weeks intervention, the fasting blood glucose (FBG), oral glucose tolerance test (OGTT), hemoglobin A1C (HbA1c) were tested as indicators of glucose metabolism, and the gut microbiota, SCFAs concentration in the ileocecal, serum glucagon-like peptide-1 (GLP-1), fasting serum insulin, and GLP-1 expression in intestinal mucosa were performed for intestinal-glucose metabolism mechanism.Results:The FBG and HbA1c were significantly increased in tacrolimus group[(7.31±0.97)mmol/L, (8.34±1.12)%] than control group [(5.23±0.30)mmol/L, (4.32±0.80)%, all P<0.05], which remained normal in antibiotic group [(4.92±0.31)mmol/L, (5.61±0.98)%)], tac&abx group[(5.95±0.37)mmol/L, (4.56±0.26)%] and SCFAs groups [(5.87±0.68)mmol/L, (5.07±1.79)%]. The OGTT in the tacrolimus group showed glucose tolerance impairment, while other groups remained normal. The ileocecal butyric acid and isovaleric acid concentrations in the tacrolimus group were (722.3±262.2) μg/g and (10.0±5.1)μg/g, lower than the control group[ (1 321.3±165.5) μg/g, (19.7±3.6)μg/g, P<0.05]. The above acids in the SCFAs group remained normal as in the control group [(1 375.7±451.6) μg/g, (24.5±11.5)μg/g)]. The fasting serum insulin in the tacrolimus group decreased significantly to (3.2 ± 0.6)mIU/L, compared with control[ (4.4±0.9) mIU/L]and SCFAs groups [(7.0±1.1) mIU/L]. The GLP-1 test indicated a significant decrease in the tacrolimus group[ (4.7±2.9)pg/ml, P<0.05] compared with the SCFAs group and control group [(42.5±19.9) pg/ml, (33.1±9.1) pg/ml]. Conclusions:Tacrolimus affects glucose metabolism through the SCFAs-associated GLP-1 pathway in the intestine, and oral supplementation with mixed SCFAs provides a new insight for the prevention and treatment of tacrolimus-induced hyperglycemia in transplant recipients.

ABSTRACT

BACKGROUND@#The development of the technique has improved the success rate of percutaneous coronary intervention (PCI) for in-stent chronic total occlusion (IS-CTO). However, long-term outcomes remain unclear. The present study sought to investigate long-term outcomes of PCI for IS-CTO.@*METHODS@#A total of 474 IS-CTO patients were enrolled at two cardiac centers from 2015 to 2018 retrospectively. These patients were allocated into either successful or failed IS-CTO PCI groups. The primary endpoint (major adverse cardiac events [MACE]) consisted of recurrent angina pectoris (RAP), target-vessel myocardial infarction (MI), heart failure, cardiac death, or ischemia-driven target-vessel revascularization (TVR) at follow-up. Multivariable Cox regression analysis was used to investigate the association between treatment appropriateness and clinical outcomes.@*RESULTS@#A total of 367 patients were successfully treated with IS-CTO PCI while 107 patients had failed recanalization. After a median follow-up of 30 months (interquartile range: 17-42 months), no significant difference was observed between the two groups for the following parameters: cardiac death (successful PCI vs. failed PCI: 0.9% vs. 2.7%; adjusted hazard ratio [HR]: 1.442; 95% confidence interval [CI]: 0.21-9.887; P = 0.709), RAP (successful PCI vs. failed PCI: 40.8% vs. 40.0%; adjusted HR: 1.025; 95% CI: 0.683-1.538; P = 0.905), heart failure (successful PCI vs. failed PCI: 6.1% vs. 2.7%; adjusted HR: 0.281; 95% CI: 0.065-1.206; P = 0.088), target-vessel related MI (successful PCI vs. failed PCI: 1.5% vs. 2.7%; adjusted HR: 1.150; 95% CI: 0.221-5.995; P = 0.868), MACE (successful PCI vs. failed PCI: 44.2% vs. 45.3%; adjusted HR: 1.052; 95% CI: 0.717-1.543; P = 0.797). More patients were free of angina in the successful IS-CTO PCI group compared with failed PCI in the first (80.4% vs. 60%, P 18 months of dual antiplatelet therapy (DAPT) was an independent predictor of decreased risk of TVR (HR: 2.682; 95% CI: 1.295-5.578; P = 0.008) or MACE (without TVR) (HR: 1.898; 95% CI: 1.036-3.479; P = 0.038) in successful IS-CTO PCI.@*CONCLUSIONS@#After a median follow-up of 30 months, the successful IS-CTO PCI group had MACE similar to that of the failed PCI group. However, the successful IS-CTO PCI group had improved angina symptoms and were free from requiring coronary artery bypass grafting in the first or second years. To decrease MACE, DAPT was found to be essential and recommended for at least 18 months for IS-CTO PCI.

ABSTRACT

The occurrence, development and outcome of most diseases are a complex process. It is the advantage of traditional Chinese medicine to establish complex interventions that are consistent with the characteristics of disease development. The specific steps are as follows: quantitative research on literature research, establishment of interview framework; focus on interviews, specificization of interview outlines; semi-structured interviews, preliminary construction of complex intervention programs; evaluation of efficacy of complex interventions. The introduction of semi-structured interviews and other qualitative research methods into the modernization of traditional Chinese medicine, combined with quantitative methods such as text analysis and data mining, is also helpful in formulating the standard of diagnosis and treatment system with Chinese characteristics.

ABSTRACT

OBJECTIVE:To compare the clinical efficacy of raloxifene and clomiphene in the treatment of polycystic ovary syndrome(PCOS)complicated with infertility. METHODS:In retrospective study,clinical information of 137 PCOS patients with infertility in our hospital during Jun. 2012-Feb. 2015 were selected and divided into raloxifene group(70 cases)and clomiphene group(67 cases)according to therapy plan. Raloxifene group received Raloxifene tablet 120 mg at the fifth day of menstrual cy-cle,qd,for consecutive 5 d. Clomiphene group received Clomiphene citrate capsule 100 mg at the fifth day of menstrual cycle, qd,for consecutive 5 d. Both groups received B-ultrasound to detect the follicular diameter and endometrial development at the twelfth day of menstrual cycle. When endometrial thickness was ≥7 mm and the diameter of one follicle was ≥18 mm at least,all patients received abdominal subcutaneous injection of Triptorelin acetate injection 0.1 mg to induce ovulation;after 48 h,ovulation was confirmed by B ultrasound. The patients with insufficient luteal function should take dydrogesterone 10 mg as appropriate,q12 h,till 15 d after ovulation. The endometrial thickness,the number of follicles with diameter≥18 mm and follicular maturation time were compared between 2 groups on the day of injection. The sex hormone levels before treatment and on the day of injection,as well as ovulation,outcome indexes and the occurrence of ADR in 2 groups were compared. RESULTS:The endometrial thickness of raloxifene group was greater than that of clomiphene group;the number of follicles with diameter ≥18 mm was more than clo-miphene group;the follicular maturation time was shorter than clomiphene group,with statistical significance(P0.05). Compared to before treatment,there was no statistical significance in serum levels of P,E2,FSH or LH in 2 groups on the day of injection(P>0.05). Serum levels of LH in 2 groups were decreased significantly,and serum level of LH in raloxifene group was significantly lower than in clomiphene group,with statistical significance(P0.05). There was no statistical significance in the total incidence of ADR (5.71% vs. 8.96%)between 2 groups(P>0.05). CONCLUSIONS:Raloxifene has better ovulation induction effect than clomi-phene,and it can improve the sex hormone levels of PCOS patients with infertility,with few ADR,and similarity in long-term clinical effect with clomiphene.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of dexmedetomidine on inflammatory reaction, oxidative stress, and renal pathologies in a rat model of lipopolysaccharide (LPS)-induced sepsis.</p><p><b>METHODS</b>Thirty-two SD rats were randomly divided into 4 groups, including a sham-operated group, LPS group with LPS (5 mg/kg) injection via the caudal vein 30 min before the operation, dexmedetomidine (Dex) +LPS group with additional Dex (10 µg/kg) injection via the caudal vein 10 min before LPS injection, and yohimbine+DEX+LPS group with intraperitoneal yohimbine (1 mg/kg) injection 40 min before and Dex injection 10 min before LPS injection. The levels of IL-1β, SOD and MDA in the plasma and renal tissues were determined, and the renal pathologies were examined.</p><p><b>RESULTS</b>Compared with the sham-operated rats, the rats in LPS group showed significantly increased IL-1β and MDA levels and lowered SOD activity in the plasma and renal tissues (P<0.05) with obvious renal pathologies. Dex pretreatment obviously lowered IL-1β and MDA levels and enhanced SOD activity in the plasma and renal tissues in LPS-challenged rats (P<0.05), and significantly lessened LPS-induced renal pathologies.</p><p><b>CONCLUSION</b>Dex can protect the rats against LPS-induced renal injury by alleviating the inflammatory reactions and cytokine oxidative stress, and this effect is mediated possibly by α2 receptors.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the differences in the therapeutic effect on child amblyopia between auricular point sticking therapy and routine complex treatment.</p><p><b>METHODS</b>Two hundreds and thirty cases of amblyopia were randomly divided into an observation group and a control group, 120 cases (212 eyes) were in the observation group and 110 cases (194 eyes) were in the control group. The observation group was treated with auricular point sticking therapy. The main points were Yan (eye), Shenmen, Gan (liver), Pi (spleen) and Shen (kidney), etc. The control group was treated with routine complex treatment, such as wearing glasses, shade therapy and family refined performance therapy. The changes of vision were observed after treatment in the two groups. The follow-up was 3 years.</p><p><b>RESULTS</b>The effective rate was 81.0% (64/79) in the observation group of ametropic amblyopia and 52.2% (36/69) in the control group. The effective rate was 73.1% (49/67) in the observation group of anisometropic amblyopia and 47.7% (31/65) in the control group. The effective rate was 71.2% (47/66) in the observation group of strabismic amblyopia and 45.0% (27/60) in the control group. The therapeutic effect of the observation group was superior to that of the control group (all P < 0.05).</p><p><b>CONCLUSION</b>Auricular point sticking therapy can obviously improve child visual acuity with simple manipulation.</p>

ABSTRACT

Objective: To study the expression of chemokine receptor CCR5 in follicular thyroid carcinoma and the serum level of CCR5 ligand, so as to assess the role of CCR5 in progression and metastasis of follicular thyroid carcinoma. Methods: Fifteen samples of follicular thyroid carcinoma, 17 samples of follicular thyroid adenoma and 12 adjacent normal samples were analyzed immunohistochemically for CCR5 expression. The sera concentrations of CCL3, CCL4 and CCL5 were measured by ELISA in all patients. Results: CCR5 was positive in follicular thyroid carcinoma samples, with the positive rate being 73.33%, and was not detected in the follicular thyroid adenoma and the normal samples (P<0.01). The concentrations of CCL3 and CCL5 in the sera of follicular thyroid carcinoma patients were significantly higher than those of the other 2 groups (P<0.05). Conclusion: CCR5 is highly expressed in follicular thyroid carcinoma tissues and the concentrations of CCL3 and CCL5 are obviously increased in the sera of patients, indicating that CCR5 may play an important role in the pathogenesis of follicular thyroid carcinoma.

ABSTRACT

Objective: To analyze the polymorphism in human cDNA sequence of prothymosin α (ProTα) by sequencing analysis. Methods: The cDNA of human ProTα was amplified from cells of peripheral blood and cord blood by RT-PCR. The product of RT-PCR was purified and linked with vector pMD18-T. After cloning and sequencing, the sequence of ProTα cDNA was compared with the standard sequence to analyze the polymorphism in the ProTα cDNA sequence. Results: The cloned ProTα cDNA sequence was different from that of the standard. We found 2 kinds of variations: (1) The nucleotide in 107 position was varied and the nucleotides in 110-121 and 191-205 positions were deleted; (2) The nucleotide in 306 position was deleted, mainly in the 60-80 years old group. Conclusion: We have identified 2 kinds of variations in human ProTα cDNA, but the first 28 amino acid in the N-terminal of cDNA of human ProTα are not involved therefore the variations do not affect the function of human ProTα.

ABSTRACT

<p><b>OBJECTIVE</b>To develop an simple and sensitive method for detecting anti-coronavirus IgG antibodies in bat sera based on enzyme-linked immunosorbent assay (ELISA).</p><p><b>METHODS</b>A commercial ELISA kit for detecting SARS-CoV antibody was modified for detecting coronavirus antibodies in bat serum samples. The second antibody in the kit was replaced with horseradish peroxidase-conjugated protein-A (HRP-SPA) based on the characteristics of binding between Staphylococcus aureus protein A (SPA) and mammal IgG Fc fragment. The sera of 55 fulvous fruit bats (Rousettus dasymallus) were tested using the SPA-ELISA.</p><p><b>RESULTS</b>The test results of the positive and negative controls in the kit and the serum samples from convalescent ;patient were consistent with expectation. Coronavirus antibody was detected in 2 out of the 55 bat serum samples. Serum neutralization test confirmed the validity of the SPA-ELISA method.</p><p><b>CONCLUSION</b>This SPA-ELISA method is applicable for detecting coronavirus antibody in bat sera.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study applied anatomy of adult mandibular ramus and condyle.</p><p><b>METHODS</b>Observation and measurement were made on 16 adult mandibular rami and condyles.</p><p><b>RESULTS</b>Thickness became gradually greater within 15 mm from the back edge of ramus. Some adult mandibular rami had no spongy bone at the level of 10 mm above mandibular foramen.</p><p><b>CONCLUSIONS</b>These date are very important for application of lag screw osteosynthesis in condylar fracture.</p>
